Library Card Application

The Niles-Maine District Library covers Niles, parts of unincorporated Des Plaines, and parts of unincorporated Glenview. If you live in these areas, please apply for a library card here. District 63 students who live in the unincorporated areas of Glenview, but do not live in the Library District, qualify for a card.

Not sure if you live in the Library District? Call Patron Services at 847-663-1234 to verify your address.

After you fill out the form, we will email you your card number within 48 hours. We will also mail you a physical library card with the same card number.

September Special: Sign up for a new library card in-person or online in September and receive a coupon for a free one topping individual pizza courtesy of Lou Malnati’s. While supplies last. Coupons will be mailed to patrons who sign up for a library card online.
